Session Description: Social isolation and loneliness have emerged as a major public health crisis with profound impacts on morbidity, mortality, and health equity. This session examines the epidemiology and health consequences of loneliness, introduces the concept of social health, and highlights Connecticut’s coordinated response through the Connecticut Collaborative to End Loneliness. Participants will explore practical, scalable strategies, such as screening, social prescribing, and community partnerships, to integrate social connection into primary care and health professions education.
 
Session Learning Objectives:  Participants will (be able to):
  1. Describe social isolation and loneliness as population health challenges, with a focus on prevalence, risk factors, and health outcomes.
  2. Describe its three vital components, the many factors that shape it, how it influences health outcomes, the six pillars to advance social connection, as it pertains to social connection.
  3. Identify the strategies being employed to encourage and equip healthcare providers to recognize social health, address loneliness, and promote meaningful social connections in patient care.
  4. Discuss the role of social prescribing, community partnerships, and referral pathways in addressing loneliness at scale.
  5. Identify opportunities to engage health professional learners in population-level interventions targeting social isolation across the lifespan.
